Transition is important for students with disabilities to succeed in school. Whether it is from grade to grade, from early childhood programs to elementary school, elementary to middle school, middle to high school, or high school to college or employment, the special education team is ready to support our students.Transition is a process of a coordinated set of activities that includes instruction, related services, community experiences, the development of employment and other post-school adult living objectives, and when appropriate, acquisition of daily living skills and functional vocational evaluation.

When should we start planning for Transition?
Transition services means a coordinated set of activities for the child with a disability that links a student to adult life; post-secondary education, training, employment, and independent living when applicable.Discussing transition from school to career and college can begin any time during the student’s education. When your child is 14 years of age, LEISD begins to formally address transition in the IEP process.
